Abstract: The new psychoactive substances (NPS) world is constantly and rapidly evolving, and this phenomenon poses a threat to public health. The signs and symptoms of NPS intoxications can be unpredictable and variable. The exact prevalence and clinical characteristics of laboratory-confirmed NPS intoxications evaluated in emergency settings are not well defined. The European Early Warning System collects data from agencies and health systems. One of the most popular emerging classes of NPS is the synthetic cathinones. Initially, the cathinones, particularly mephedrone (4-methylmethcathinone), were sold undisguised by their chemical names, but they are also, like many other NPS, sold over the Internet and in “head shops” as “research chemicals,” “bath salts,” or “plant food,” often with a warning “not for human consumption” or “not tested for hazards”. This marketing is an attempt to circumvent existing local, national, or international medicinal and/or drug laws. Recently a new practice in which drugs are abused to improve sexual performance (Chemsex) is increasing in MSM/transgender communities. Substances more commonly used are cathinones, GHB/GBL, popper, MDMA, ketamine, sildenafil, usually ingested, smoked, sniffed or injected. High variability in clinical pictures and in the different substances abused will be described. In cases without accordance between declared substances, a second level toxicological analysis may be required. The laboratory able to perform second level analysis gives a fundamental help to physicians to make diagnosis and to identify new molecules, especially when cathinones are involved.
